SportsPlus accepts most major types of insurance. If you have any questions about coverage, please contact your provider prior to your first office visit. Our staff will also be happy to help you with processing insurance claims and providing information.

To visit our practice and to receive insurance benefits, you do not need to have a referral from your medical doctor. Coverage or reimbursement, less your required co-pay amount, will be allowed by your insurance company.

Remember to bring your insurance card with you to your appointments!
