We welcome all new patients! We understand that injuries can occur at any time, and when you are in pain or concerned about your condition, you need help right away. Walk-in appointments are available, as are scheduled appointments made in advance.

New Patients

At your initial visit, we will need some patient information. Just click on the link below, print out the form, fill it in, and bring it with you to your first appointment. You can also complete the paperwork in our office. Please wear comfortable clothing and remember to bring your insurance card with you.

Consultation and Examination

Your first visit will include a consultation with Dr. Le Cara or Dr. Winter to discuss your sports-related injury or health concern and/or reasons for your visit. They will review your health history, perform an initial examination and analyze your injury or condition. They will assess, diagnose and recommend a comprehensive treatment plan for you.

Treatment

Drs. Le Cara and Winter use an evidence-based approach to treatment; i.e., your course of therapy will be specifically designed for you and recommended based on 1) your health history, 2) your recent or past injury or injuries, and 3) the most progressive therapies, as published in the most current scientific and medical literature.

Before treatment begins, the types of improvements you can expect to see and feel, as well as when you can expect to resume physical activity, will be discussed. Your treatment regimen may begin during your first visit, so be sure to wear comfortable clothing.

You may require other healthcare services, such as nutritional counseling, massage therapy, physical therapy, or personal or athletic training. All of these services are available within the Sports Plus practice. If medical attention (surgical, pharmacological) is required for your particular injury, you will be referred to a SportsPlus-affiliated Medical Doctor for treatment.
